Services Available/Provided/Purchased
The Polk Co Senate Bill 40 Board has contracted with the Center for Human Services to provide Targeted Case Management Services for individuals in Polk County. Our website is www.chs-mo.org

Individuals must have a developmental disability which is attributable to a mental/intellectual or physical impairment which occurred before the age of 22. It must be determined that the disability is likely to continue indefinitely AND that it results in a substantial functional limitation in two or more major life activities.

The Springfield Regional Office determines if someone meets the eligibility criteria. Once the Regional Office determines the person is eligible, they refer the individual to the Center for Human Services where a Service Coordinator will be immediately assigned and contact made within 24 hours.

Board of Directors Meetings
Board meetings take place quarterly on the second Thursday’s at 6pm (January, April, July and October) and are held at the Center for Human Services Polk County Service Coordination Offices at 4746 S. 131st Rd, Unit C, Bolivar, MO 65613
